Use this registration for the Life Support Education team to create an RQI account.  You must register with your CHOP employee ID, CHOP email address (@chop.edu), role/title and cost center name/department (please don't write Nursing or NCCS).  If you don't know your cost center name/department please ask your direct supervisor.

Please indicate which certifications you need to have on your RQI profile.  

BLS/CPR can be for first time or renewals

If you are requesting PALS or ACLS you must email lifesupportcenter@chop.edu copies of your current certifications

If you need to take PALS or ACLS for the first time or you are expired please register for  Provider course

There is NO PEARS RQI
